 March is recognized globally as Colorectal Cancer Awareness Month, a time dedicated to increasing awareness about prevention, early detection, and life-saving screening. While screening procedures such as colonoscopy play a crucial role in detecting early disease, diagnostic imaging is equally vital in the detection, staging, and management of colorectal cancer

From a clinical perspective, imaging provides critical information that guides treatment decisions and helps clinicians deliver more precise and effective care throughout the patient’s cancer journey. 

Why Imaging Matters in Colorectal Cancer

Colorectal cancer can develop insidiously, often progressing without noticeable symptoms in its early stages. When patients do present with symptoms — such as rectal bleeding, unexplained weight loss, persistent abdominal pain, or changes in bowel habits — imaging becomes an essential component of the diagnostic process.

Medical imaging assists clinicians in:

  • Confirming the presence of a suspected malignancy 
  • Determining the extent of disease (staging) 
  • Guiding treatment planning and surgical decision-making 
  • Monitoring response to chemotherapy or radiation therapy

Key Imaging Modalities in Colorectal Cancer

Perianal MRI

Advanced Perianal MRI is an important imaging tool for evaluating complex perianal conditions and disease involvement in the anal canal region.

  • Perianal fistulas 
  • Abscess formation 
  • Tumor involvement of the anal canal and surrounding tissues

Accurate imaging of these structures allows for precise mapping of disease, reducing surgical complications and improving overall treatment outcomes.

PET/CT Imaging

PET/CT combines metabolic imaging with anatomical imaging, allowing clinicians to detect areas of increased metabolic activity often associated with cancer.

This modality is particularly useful for:

  • Detecting metastatic disease not seen on conventional imaging 
  • Evaluating suspected recurrence 
  • Assessing treatment response in advanced cases

Ultrasound

In selected cases, endorectal ultrasound may be used to evaluate early-stage rectal tumors and determine the depth of tumor invasion within the rectal wall.
